Complete the following information.

All new drugs* require prior approval until the D and T Committee recommends that they be made available without prior approval. Even if a new drug is included in a PDL class, the D and T Committee must approve the product as a new drug before it can be made preferred.

Requests to be on the agenda must be submitted at least 3 weeks prior to the posted meeting date. If the request is not received at least 3 weeks prior to the meeting date, the drug will be added to the agenda of the subsequent meeting.

One appeal is allowed per year per product.

The drug must be available in the marketplace before being reviewed by the D and T Committee.

*All agents for which the primary indication is treatment of HIV/AIDS (antiretroviral agents) or Cancer (oncolytic agents) are available without prior approval. The D and T Committee does NOT review these products.

The D and T Committee does NOT review non-drug products. Examples of these types of products include nutritional supplements, medical foods, medical devices, diagnostic tests, and supply items.

Once your request is received and reviewed, you will receive a confirmation from Ken Ryan that your product is on the agenda. Products under review are posted at Committee on Drugs and Therapeutics.
