  HFS 1802 Healthy Kids Annual Notice

 

Healthy Kids need checkups for preschool, Head Start, school, camp, childcare, sports programs, WIC and just to stay healthy!

Having All Kids or a MediPlan card means you can take your child to the doctor for a checkup, to the dentist to fill a cavity, or to the eye doctor to get a prescription for glasses. It means they can get all the care they need to stay healthy.

Babies, kids and teens can get FREE checkups with their All Kids or MediPlan cards. Children with disabilities who have a MediPlan card can get free checkups through age 20.

Healthy Kidscheckups include:

  • Head to toe exam
  • Checks for growth and development
  • Immunizations (shots)
  • Checks of eyesight, hearing and teeth
  • Advice about healthy eating
  • Answers to your health questions
  • Checks of emotions and behavior
  • Referrals to special services if your child needs them

If you have questions about checkups for your child, call your doctor. If you need help to find a doctor or make an appointment, call Illinois Health Connect at 1-877-912-1999 (TTY: 1-866-565-8577). If you live in Cook, Madison, St. Clair, Washington, Randolph, Perry, Jackson or Williamson counties, call the Illinois Client Enrollment Broker at 1-877-912-8880 (TTY: 1-866-565-8576). The call is free. Or, you may call your managed care organization (MCO) if your medical card shows that you are enrolled in an MCO.

Remember, even young children need good dental care so they do not get cavities or toothaches. Take your child to a dentist regularly starting no later than age one. If you need help to find a dentist, call 1-888-286-2447 (TTY: 1-800-466-7566). The call is free.

If your child is enrolled in All Kids Assist, All Kids Share, All Kids Premium Level 1 or has a MediPlan card, you may get help with transportation to your doctor or dentist. If you have an All Kids card, check the back of the card to find your child’s All Kids plan. Call 1-877-725-0569 (TTY: 1-877-204-1012) to find a transportation provider. The call is free.

Be sure to use all the benefits your MediPlan or All Kids card covers for your children.
