Your Healthcare Plus™ at 1-800-973-6792 is a free, voluntary program that helps people with ongoing health problems. If you or your child have asthma, or if you have diabetes, heart disease, chronic obstructive pulmonary disease, difficulty breathing, lung problems, coronary artery disease, mental illness or other health problems, this program may help you to:
- Stay healthier
- Understand your disease better
- Understand more about your medicines and how to take them
- Work better with your doctor or clinic
Everyone in Your Healthcare Plus gets health information in the mail and has access to a free Web site. Depending on your health, you may also have visits with a registered nurse or someone to help you with other services or follow-up care after you’ve been in the hospital.
Your Healthcare Plus is part of your Illinois healthcare program. You do not pay anything to be in this program. You can decide if you want to be in this program or not.
Your Healthcare Plus does not replace or change what is covered by your health program. The program staff work with your doctor or clinic to help you stay healthy and feel better. It is important for you to keep all your doctor's appointments. Your Healthcare Plus can help you find a doctor if you do not have one.
In this Web site you will find Action plans, which help you know what your doctor wants you to do if you're not feeling well. These action plans can be printed and taken with you each time you see your doctor. There are also some brochures that tell you about the Your Healthcare Plus program and special health problems you may have.
